Job Description:

I. GENERAL STATEMENT OF RESPONSIBILITIES To provide needed and appropriate intensive wraparound services to delinquent youth and their families. To coordinate the delivery of collateral resources as required by State of Michigan laws and applicable Service Standards for the purpose of increasing the youth's ability to function successfully in the community at-large. II. QUALIFICATIONS

  • Education: Bachelor's Degree in social work, sociology, psychology, or Guidance and counseling or a master's degree in sociology, psychology, social work criminal justice, or guidance and counseling.
  • Experience: One year post degree experience with youth and families.
  • Transportation: Reliable and ongoing as required to make home visits and
  • Provide transportation to youth and families. Valid unrestricted Michigan driver's license. Good driving record. III. SPECIFIC DUTIES AND RESPONSIBILITIES DIRECT SERVICES
  • Attends and admission conference or transition conference and completes the placement process as assigned, providing a smooth transition for the youth and the youth's care givers by obtaining and providing necessary information and documents and by making in-person contact within 24 hours of placement.
  • Completes an intake assessment and initial plan of care and community based treatment plan within 30 days of youth's return to the community/admission to the program.
  • Obtains appropriate releases and authorizations within time frames required from the family during the initial family interview.
  • Provides direct services as needed to include transportation to ensure continuous agency supervision to youth and families in accordance with IPOC/ITP/UTP and as required by funding bodies and Services Standards.
  • Performs case management duties on another caseload as assigned by the supervisor.
  • Keeps supervisor fully informed of case progress and case status orally and in writing as required; meets with supervisor individually, in Treatment Team meetings and in other meetings as may be required.
  • Develops and leads treatment groups for youth.
  • Travels to various locations (homes, school, employment, contacts, and community resources) to facilitate the treatment plan.
  • Participates in a system of 24-hour staff availability to clients in crisis.
  • Protects confidentiality appropriately, sharing information for the benefit of the client, and obtains releases of information as necessary.
  • Makes appropriate referrals and secures needed services from collateral agencies and community services.
  • Makes appropriate referrals to Protective Services according to the Child Protection Law, fulfilling all professional responsibilities and obligations.
  • Reads, stays familiar, and complies with all current Service Standards applicable to the intensive home-based programs and adjudicated youth, e.g., Care Management Organization Manual Items, Program Manual, Master Contact.
  • Adheres to Wayne County Department of Child & Family Services policies and CCMO Personnel Policies & Procedures.
  • Adheres to the Third Judicial Circuit Court System Policies and Procedures.
  • Provides timely completion and submission of Individual Utilizes the risk/needs on developing the Initial Treatment Plan of Care (POC) for each enrolled youth. Case Management Duties
  • Obtains and or completes required case documentation at intake and ongoing, such as social histories, school records, court orders and reports, reports of examinations, and reports from collateral services providers.
  • Secures or advocates for appropriate educational or vocational placement for each youth by participating in IEPC, school conferences, and meeting with school personnel.
  • Maintains accurate records within specified time frames, e.g., update Service plans, treatment plans, narrative notes, school reports, medical and dental examination, petitions, graduation summaries, referrals for collateral services, psychological or psychiatric examinations, contact sheets, level system as required by funding bodies, Service Standards, agency or program policies or procedures.
  • Provides required reports to the Juvenile Court, Care Management Organization, Department of Mental Health or other agency approved organizations by the required due dates.
  • Keeps all members of the Treatment Team, collateral service keeping the funding agencies informed of case progress, changes in case situation or individual treatment plan, working cooperatively with all affected Spectrum programs collateral agencies or funding sources.
  • Provides statistical reports as required, e.g., number of youth in services, number and type of contacts, etc.
  • Prepares for and attends on time all required meetings, such as Treatment Team Unit, Case Assessment, Intake, Educational Planning, etc.
  • Actively participates as a team in maintaining the treatment team.
  • Develops and maintains positive working relationships with providers and Juvenile Court staff.
  • Displays professional courtesy by being on time for home visits, appointments and meeting, canceling or rescheduling ahead of time if possible, and arranging for coverage during absences or illness.
  • Assists in the maintenance of the Management Information Systems by providing information, entering data on the computer, and generating reports as required.
  • Provides or arranges individual and family therapy as needed based on the court order and treatment plan.
  • Conducts termination interviews and completes termination reports using the format provided by the CMO and submits it to the CMO Case Manager on a timely basis.
  • Makes recommendations to CMO Case Manager for youth whose behavior is deteriorating and is in need of placement.
  • Completes the assessments and reassessments of a juvenile risk level and the development of a corresponding plan for case management contacts, to the JAC.
  • Makes recommendations to the court to terminate enrollment for accepted cases.
  • Maintains a caseload.
  • Provides a day-to-day case management services to all assigned clients and their families, linking and coordinating the child and family with all services to same providers (recognizing that the choice of provider rests with the client and their satisfaction with the provider).
  • Monitors all services to ensure that services have been delivered as arranged and that the client's needs are being met.
  • Maintains regular contact with youth, family and service providers based on established contract schedule; adjusting schedule as needed.
  • Receives and documents daily reports from provider agencies on all youth non-compliance issues. Develops a plan to manage the issues.
  • Monitors length of stay in detention, residential and community based program.
  • Responsible for completing preliminary investigations, reports and timely and accurately documented.
  • Responds to client issues and concerns 24 hours a day, 7 days per week, via the program director/supervisor or designee.
  • Informs youth/parent/guardian of scheduled psychological, psychiatric and academic evaluations.
  • Arrange transportation for youth to academic, psychological and psychiatry evaluations, as needed.
  • Provides timely and accurate notification to the Warrant Enforcement Bureau.
  • Arranges and monitors drug screenings and follow-up services as needed. Court
  • Prepares documentation to file Violations of Probation, Writs of Apprehension and court reports.
  • Represents the program at all court hearings, giving testimony, making recommendations and complying with court protocol.
  • Implements court orders as required.
  • Educates and prepares youth and families for the court process.

  • Attends and participates in all required or assigned training and conferences.
  • Works cooperatively with other agency staff, serving on committees, completing special projects, and participating in agency decision making as needed.
  • Represents the agency's programs and services in the community through interagency committees and organizations, and public speaking.
  • Complies with all Spectrum Personnel Policies and Procedures, and all program policies and procedures.
  • Completes all tasks and duties as may from time to time are assigned by Spectrum administrative staff.
  • Maintains assigned manuals by filing new material and keeping in the Office; uses keys, gate cards, identification cards, beepers office equipment and supplies, and all other agency property in good repair upon termination of employment.
  • Reads, stays familiar and complies with all current Services Standards applicable to the intensive home based programs and adjudicated youth, e.g., Care Management Organization Manual Items, Program Manual, Master contract.
