Pay Information $2,092 per week About The Position Job Title: Registered Nurse Profession: Registered Nurse (RN) Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: 2 years in Case Management, 3 years in acute hospital care License: State License Certifications: BLS (AHA), American Case Management Certification (ACM) required, willing to obtain prior to starting Must-Have: - 2 years in Case Management - 3 years in acute hospital care - BSN - American Case Management Certification (ACM) Description: Performs care coordination assessments for the initial assessment of patients within 24 hours of admission. Conducts assessments for readmission and transition planning. Works collaboratively with social workers and other disciplines to ensure a safe, appropriate, and timely transition to the next level of care, taking into consideration the patient’s available resources. Assesses patient and family needs to reduce barriers and formulate discharge plans, including length of stay barriers to discharge. Identifies unsigned level of care orders and communicates with the utilization management nurse to obtain orders from providers. Reviews current DRG/LOS identified within the electronic medical record system to assess discharge planning needs with providers and identifies the designated family member point of contact. Assesses the risk of readmission for specified patient populations and initiates assigned interventions that will enhance the patient’s ability to successfully transition along the care continuum. Performs discharge planning coordination and referral by making appropriate referrals to social services, ancillary departments, outpatient case management, durable medical equipment, post-acute placement, and other outside agencies according to Standard Operating Procedures. Acts as a liaison by collaborating and communicating daily with physicians, patients, families, nursing, and other members of the healthcare team. Actively participates in clinical case review and rounds with the interdisciplinary team. Documents in the electronic medical record assessments, plans, interventions, barriers, and reassessments to facilitate discharges and transitions, manages anticipated discharge dates, and ensures all pertinent information is transferred to post-acute agencies. Identifies barriers early in the patient’s stay and formulates a plan with the patient, family, internal and external members of the healthcare team, payers, and community resources. Identifies and reports avoidable days, variances, or service delays from the established plan of care to leadership. Represents the integrated care management department on various teams and performance outcomes committees and projects. Ensures that patients' follow-up appointments with primary care providers have been made prior to discharge. Maintains effective operations by following policies and procedures.
