high-quality, patient-centered care. We are seeking a compassionate
Speech Language Pathologist (SLP) for a contingent home health
position, ideal for a quick learner who thrives in a team
environment. Responsibilities: Provide speech pathology and
audiology services per physician’s treatment plan, accepting only
referrals from designated care managers. Assess patient
functioning, develop and revise care plans, and set treatment
goals. Document patient responses and communicate changes to the
care team. Educate patients, families, and health personnel on
speech therapy procedures to promote independence and disease
prevention. Recommend hearing tests and check hearing aids when
needed. Requirements: Active Michigan Speech Therapy license.
Reliable transportation, valid driver’s license, and auto
insurance. Home health care experience preferred. Compensation:
Visit rates range from $63.00 to $105.00 per visit, with mileage
reimbursement at $0.50 per mile after 30 miles/day, and additional
pay for orientation and in-service hours. Work Environment:
On-the-road visits and office meetings, with participation in
in-service programs, requiring dependable transportation and auto
insurance.
