The Director of Social Services - Licensed is responsible for
overseeing social services provided by the department to short term
rehabilitation, long term care residents; in addition to providing
direct case management to residents residing at the facility. They
evaluate patients and identify need areas by coordinating and
directing other social workers in carrying out their job duties.
Additionally, coordinates services with the interdisciplinary team
and contacts community resources as needed. The Director of Social
Services - Licensed oversees case management services for patients
and reviews patient status. The position is supervised directly by
the Administrator and does administratively supervise other
positions when specified per center. Strictly follows policy and
safeguards computer passwords and computer use authorities.
Understands the importance of logging on and off the computer
clinical record system and will consistently maintain all computer

· Coordinate social services with patient, staff, family
members, and significant others as applicable. Provide social
services as specified in the patient plan of care and consistent
with treatment goals.

accepted standards of social work and all applicable federal,
state, and local laws and regulations and interpretations
· Provide specialized training to other staff, family members,
consultants, and other individuals as needed. Participate in
clinical case conferences and in-service programs to share
information with the interdisciplinary team.
Education: Minimum of a Bachelor Degree in social work or social
welfare from an accredited academic institution. Preferable a
Masters. Possession of a current state license as a Social
Experience: Experience in outpatient medical rehabilitation
